In the new millennium, humans will be traveling to Mars and perhaps beyond with skeletons that respond to microgravity by self-destructing. Meanwhile in Earth’s aging populations growing numbers of men and many more women are suffering from crippling bone loss. During the first decade after menopause all women suffer an accelerating loss of bone, which in some of them is severe enough to result in “spontaneous†crushing of vertebrae and fracturing of various bones even by ordinary body movements. This is osteoporosis, which all too often requires prolonged and expensive convalescent care and leads to the further bone loss caused by immobilization, the incapacitation and mental stress of which may even kill the patient. The ominously accelerating microarchitectural deterioration and with it rising microfracturing by the still relatively large muscle pulling of normal activities in aging postmenopausal women is caused by the loss of estrogen. The slower development of osteoporosis in aging men is also partly due to estrogen made in ever smaller amounts in bone cells from the declining level of circulating testosterone. It is needed for bone maintenance just as it is in women. The loss of estrogen triggers a vicious cycle of a disproportionately rising resorptive arm of the coupled resorption -> refilling microcrack-repairing mechanism and a consequently rising fragility and increasing activity-driven microfracturing. The Journal of Hand Surgery publishes original, peer-reviewed articles related to the diseases and conditions of the upper extremity. Both clinical and basic science manuscripts are considered. The journal also publishes Case Reports, which enhance the understanding of the diagnosis, treatment and pathophysiology of conditions of the upper extremity. Special features include Department of Technique articles, which emphasize the technical aspects of surgery, and Clinical Perspective reports, which place current controversial topics in their proper perspective.

An earlier study1 found genetic conditions were present in greater than 50% of patients admitted to a pediatric orthopaedic hospital with musculoskeletal disorders. The genes responsible for Mendelian inherited orthopaedic conditions, including many types of skeletal dysplasias, osteogenesis imperfecta, and neurocutaneous syndromes (neurofibromatosis) are known. For other, more common, conditions with complex inheritance patterns, such as scoliosis and osteoarthritis, the identification of genes that influence disease susceptibility has been more elusive.

Because of the recent explosion of papers submitted to the Journal of Shoulder and Elbow Surgery, the time from acceptance of a paper to its appearance in print has grown to between 8 to 10 months on average. Since we publish issues bimonthly, not monthly as do some, and by contract have a limit on the number of print pages we can use annually, this increase in excellent submissions has contributed to the delay in publishing accepted papers. We realize that this time frame is too long and have implemented several policies to reduce it to no more than 6 months, preferably less.
To make space for original articles in print, Case Reports and Letters to the Editor appear only online. We have also limited papers to no more than 12 typed pages for Original Research and 6 pages for Case Reports. We published a special issue on Arthroplasty earlier this year and plan another on Soft Tissue Disorders early next year. In addition, the Board of Trustees of the JSES has authorized us to purchase up to 100 additional pages beyond our current contractual limits in order to get more accepted articles in print over the ensuing months.
